25 | Licensing Program Analyst (LPA) Soleil Marx and Licensing Program Manager (LPM) Natalie Dunaway, met with Applicant, Rahul Bahl, for the purpose of an announced pre-licensing change of ownership inspection. Applicant requests an infant license to serve 60 infant children from birth to two years of age. The program will operate Monday through Friday from 8:00 AM to 5:00 PM. The fire clearance was granted on 04/18/23.
LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, Effects of Lead Exposure, car seat poster, menus, and daily schedule. Applicant stated the Needs and Services plans will be kept in the classrooms.
This facility provides AM/PM snack and lunch for toddlers and formula for infants through the CACFP (Federal Food Program).
INDOOR ACTIVITY SPACE:
There is 5 infant classrooms #1-#5, on the second floor of facility. The rooms are named Foxes, Owletes, Butterflies, Chipmunks, and bunnies. There are stairs, an elevator, and an evacuation sled available to evacuate the children in case of an emergency. Director stated that staff are trained for emergency and facility has an evacuation plan in place. LPA observed a sufficient number of tables, chairs, and cubbies for 60 infants. LPA observed 11 cribs and a sufficient number of napping cots/mats. LPA observed a variety of age-appropriate toys and equipment. Applicant acknowledges there will be a maximum of 11 infants under 12 months old, due to available cribs. There is a first aid kit in each of the classrooms. Medications will be stored in each classroom out of reach and inaccessible to children in care. LPA observed cleaning disinfectants are appropriately stored and inaccessible to children. Applicant stated there are no poisons or firearms on the premises. LPA observed water dispensers and water bottles for uncontaminated drinking water inside and outside. LPA observed a functional carbon monoxide detector within the facility. LPA observed facility uses full legal signatures for sign-in/sign-out procedures. |
