13 | On 5/11/23, Licensing Program Analyst (LPA) Priscilla Zamudio arrived at the facility to conduct an unannounced follow-up complaint inspection. The purpose of the inspection was to deliver investigation findings. LPA met with Assistant Director, Elizabeth Calderon and a census was taken.
During the course of the investigation, LPA conducted interviews and reviewed facility records. Based upon observations, and information gathered, the preponderance of evidence standard has been met, therefore the above allegation is SUBSTANTIATED.
Per California Code of Regulations, Title 22, Division 12, Chapter 1, this deficiency is being cited on the attached LIC 9099-D.
An exit interview was conducted with Assistant Director, Elizabeth Calderon. A copy of this report and appeal rights were provided. A Notice of Site Visit Form was provided to be posted on parent's board and must remain for 30 days.
