On 11/05/2024, Licensing Program Analyst (LPA) MIguel Herrera arrived at the facility to conduct a Case Management - Annual Continuation inspection. This report is a continuation of the annual/random inspection conducted on 11/01/2024. LPA met with Director, Yvette Stephens and a census was taken. LPA Herrera completed the CARE Tool during today's inspection as the there was a saving error during the previous annual visit that was conducted on 11/01/2024.

Exit interview conducted and report was reviewed with Director Yvette Stephens. Appeal rights were provided. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, there are no deficiencies being cited during today’s inspection. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
