25 | On 9/30/2024, an unannounced site visit was made by Licensing Program Manager (LPM) Cynthia Brannon and Licensing Program Analysts (LPA)s Brandon Tate and Behatriz Gonzalez. During today's visit, LPM and LPAs wer met by Site Director Debbie Rodriguez. The reason for today's visit was discussed and a census was taken. Today's census totaled 11 children present and the reason for today's visit was to observe the new flooring throughout the dining room, front office, staff break room, staff restroom, kitchen, and laundry room. Tile was grouted properly with commercial grade base boards throughout. LPAs discussed the possible hazards of moisture underneath the toilets without proper silicone grout around the base of toilets. Director stated she understood and would relay the information to her maintenance crew. LPM and LPAs toured the outside of the facility and the classrooms. It was discussed during today's inspection that proper visual supervision should be maintained at all times while children play outside on the play structure. Due to the play structure having tunnels, there is a risk of inadequate supervision. Director stated that she has two staff outside on either side of the play structure to assure constant supervision and in the case of an emergency, staff have radios to ensure proper coverage in supervision. LPAs discussed that the outside phone panel should be repaired, due to the inability to properly close. LPAs also discussed minor stucco damage that should be fixed in the outside play area as well as the cinder block wall in the outside play area was leaning with Site Director. LPM observed wood rot on the roof outside and it was brought to the director's attention. There were 2 doors in the children's classroom that were being repaired and posed a potential hazard to children in care. LPA discussed with Site Director, that the maintenance crew could potentially place a plastic barrier around the door frame to prevent children from sticking their hands in the gaps.
Upon LPM and LPAs observations of flooring repairs, the floors were repaired sufficiently and pose no hazards to children in care.
Exit interview was conducted with Site Director Debbie Rodriguez. A copy of LIC 809, notice of site visit, and appeal rights were given. |
