25 | On October 15, 2025, Licensing Program Analyst Miguel Herrera conducted an unannounced Case Management Inspection. LPA met with Lead Teacher, Kathryn Hess. LPA toured the facility, and a census was taken. The purpose of today's inspection was regarding an Unusual Incident that was self reported to the Fresno Childcare Regional Office, the incident occurred on 10/10/2025. The incident involved a child sustaining an injury while playing in the facility's outdoor play structure that needed medical treatment.
During today’s visit LPA Herrera conducted facility observations, interviewed staff, and reviewed facility records pertinent to the incident. During the facility tour LPA Herrera conducted observations in the outdoor play area where the incident occurred. LPA Herrera observed that the play structure was age appropriate. The play structure was maintained in a safe condition and was free of hazards. Furthermore, LPA Herrera did not observe any tripping hazards in the playground area that could pose a danger to children in care.
Based on interviews, the child lost their footing while attempting to get into the play structure's ladder. The child sustained an injury to their left arm from the impact. Per interviews, staff immediately provided aid to the child and the child's authorized representative was notified immediately.
During today's inspection LPA Herrera observed that the facility practiced proper supervision of children. Based on the discussions today, the facility practiced zoning in the outdoor space to ensure children were always supervised and that there was a direct view of each zone in the outdoor space area. Furthermore, it was an isolated incident and staff took appropriate measures to address the incident, following appropriate policies, regulations, and reporting requirements. Furthermore, based on interviews LPA Herrera determined that the facility was within ratio and was practicing proper supervision during the time of the incident. Teacher/child ratios are maintained, and adequate supervision is being provided during this visit.
Exit interview conducted and report was reviewed with Lead Teacher, Kathryn Hess. Appeal rights were provided. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, there are no deficiencies cited during today’s inspection. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
