An Informal Conference was conducted on November 06, 2024, at 10:00 AM at the Fresno Regional Child Care Office. In attendance today were Licensing Program Managers (LPM’s), Juvenal Moctezuma, Luisa Gavoutian. Licensing Program Analyst (LPA) Julio Rodriguez, Principal Nayree Shishmanian, and Site Supervisor Talar Hovagimian. The purpose of the informal conference was to discuss previous deficiencies cited. LPM’s reviewed the deficiencies with facility representatives and discussed how the Child care Center (CCC) will maintain compliance with the deficiencies cited below.

The Department of Social Services, Community Care Licensing Division has determined that, Charlie Keyan Armenian Community School has violated the following Title 22 licensing regulations/California Health and Safety Codes:

The following type A violations were discussed:

· 09/25/2024 CCR 101221(a)(3) Personal rights: Children were left in time out in a classroom by themselves and staff was observed speaking inappropriately to and roughly grabbing children in care, which are immediate threats to the health and safety or personal rights to children in care.

· 07/07/2023 CCR 101170(e)(1) Criminal Record Clearance: During file review and interviewing staff it was confirmed that staff present on this day, did not have a fingerprint clearance nor were associated to the preschool license which poses an immediate health, safety, or personal rights risk to persons in care.



The following Type B violations were discussed:

· 09/25/2024 CCR 101229 (a)(1) Responsibility for Providing Care and Supervision: This requirement was not met as evidenced by staff not supervising preschool children in restrooms and leaving children in back classroom while all staff are in the front classroom, which poses a potential threat to the health and safety or personal rights to children in care.

· 09/26/2023 HSC 1596.8662(b)(1) Administration of Child Day Care Licensing: A staff member had not completed the mandated reporter training which poses a potential health, safety, or personal rights risk to persons in care.

· 09/26/2023 CCR 101229.1(b) Sign in and Sign out: LPA observed in 2 classrooms that 6 parent signatures were missing from the sign in sheet which poses a potential health, safety or personal rights risk to persons in care.

· 09/26/2023 CCR 101216.3(b)(1) Teacher-Child Ratio: LPA observed 19 children in Advanced Preschool classroom with 1 teacher and 1 aide. Upon reviewing files LPA could not find any units for the aide. LPA spoke with aide and aide stated she did not have any units; therefore, they are over ratio which poses a potential health, safety, or personal rights risk to persons in care.

· 07/07/2023 HSC 1596.866(b) Additional health and safety training; condition of licensure: Based on interviews & records review It was confirmed that both staff present on this day’s inspection did not comply with the section cited above since they do not have a CPR First Aid Certificate which poses a potential health, safety, or personal rights risk to persons in care.


Plans of corrections have been submitted for above mentioned deficiencies.



In accordance with the regulations cited above, Licensee has agreed to the following:
  1. Licensee agrees to comply with Title 22, Division 12, Chapter 1 of the California Code of regulations & The Health & Safety Code.
  2. Licensee will ensure children are provided with a safe and healthful environment, and comfortable accommodations furnishing, and equipment at all times.
  3. Licensee understands that any form of discipline or punishment that violates a child's personal rights as specified in Section 101223 shall not be permitted regardless of authorized representative consent or authorization.
  4. Licensee will maintain visual supervision at all times to ensure the health & safety of all children in care.
  5. Licensee will ensure that all individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing, or volunteering in a licensed facility: Obtain a California clearance or a criminal record exemption.

6. The Licensee will ensure that all staff, volunteers, and child files are complete and include the necessary LIC forms, training records, education, and experience documentation.
7. Licensee understands that A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation & meet the teacher child ratio at all times.
8. Licensee understands that she can contact Community Care Licensing, if she has any questions or concerns regarding the Operation of their Facility.

Licensee was informed that any further repeats of the above deficiencies will result in a Non-Compliance Conference and possible referral to the Legal Division for Administrative Action. Furthermore, Licensee understands that any repeats of the above deficiencies within 12 months will result in the issuance of civil penalties.

Licensee will stay in compliance with CCC Title 22, Division 12, Chapter 1 regulations at all times. A signed copy of this report and appeal rights were provided to Facility Representatives. No Deficiencies were cited during today’s Office Visit.
