On 9/22/22 Licensing Program Analyst (LPA) Joseph Pacheco arrived at the facility to conduct a Case Management inspection. The reason for the inspection was to follow up on multiple unusual incidents that were reported to the Fresno Child Care Regional Office.

On 7/29/22 facility reported that Child #1 was not feeling well. Staff took child's temperature and it was 99 degrees. Child #1's authorized representative was contacted. Child #1's authorized representative advised to give child over the counter fever reducing medication. Staff went to retrieve the medication when Child #1 started having a seizure. Child #1's authorized representative and emergency medical personnel were contacted. Child #1 was taken for further medical examination. Child #1 returned to care the following business day and has had no further incidents.

On 8/11/22 facility reported that Child #2 was playing on the outdoor play structure. Child #2 just finished sliding down the slide and was running over to the steps when their foot slipped causing them to fall forward and hit the side of their face on the steps. Staff #1 who witnessed the incident immediately rushed over to care for Child #2. Staff #1 observed a wound with blood on the right side of Child #2's face. Staff #1 immediately began applying pressure to the wound and brought Child #2 inside to the Director's office. Staff continued applying first aid to the wound. Director, Jeni Stevens contacted Child #2's authorized representatives who picked up Child #2 within minutes and took them to their primary care doctor for further medical examination. Child #2's wound was treated with stitches. Child #2 returned to care the following day and has had no further issues. During today's inspection, LPA observed the play structure and cushioning where the incident took place. LPA did not observe any tripping hazards in the cushioning nor any sharp, loose or pointed parts on the play structure.

On 8/31/22 facility reported that on 8/30/22 Child #3 was dancing with other children inside the classroom when they lost their footing and fell hitting the left side of their eye against a table.

Child #3 suffered an open wound but there was no bleeding. Staff tended to Child #3 wound and contacted their authorized representative. Child #3's authorized representative picked up them up and took them to the doctor for further medical examination. No further medical treatment was required. LPA observed the area where the incident took place and did not observe any tripping hazards nor any sharp, loose or pointed parts on the table.

These appear to be isolated incidents and staff took appropriate measures to address each child's injury, following appropriate policies, regulations and reporting requirements.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.

Exit interview conducted and report was reviewed with Director, Jeni Stevens.

This report shall be made available to the public upon request. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
