25 | On 12/12/2023, Licensing Program Analysts (LPAs) Pa Kou Vue and Aurelio Mendoza conducted an unannounced case management inspection. LPAs met with Director Charlene Nolan who accompanied LPAs during a tour of facility both inside and outside and a census was taken. LPAs observed 6 preschool (PS) children + 1 teacher and 0 school age (SA) children in SA classroom, Kool Kids. LPAs explained the purpose for the inspection.
On 11/21/2023 from 8:30AM – 1:30PM, LPA Pa Kou Vue conducted an unannounced annual inspection and observed in SA classroom 7 SA children + 1 teacher and 10 PS children + 1 teacher. Based on the Director interview, it was confirmed there was commingling between PS and SA children in the SA classroom for the past 9 years.
LPA informed the Director of the intended purpose of a combination center and CCR 101538.3(b). The Director stated she understood and will ensure there is no commingling between SA and PS children. In addition, the Director stated she will review CCR 101538.3(b) with staff members and submit a statement of understanding stating how the facility will prevent commingling in the facility.
Director Charlene Nolan was provided a copy of appeal rights. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days. Exit interview conducted and report was reviewed with the Director.
Per the California Code of Regulations, Title 22, Division 12, Chapter 1, the following deficiency is being cited during today’s inspection. (See next page, LIC809-D).
This report shall be made available to the public upon request.
Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
