On 12/18/2025, Licensing Program Analyst (LPA), Ka Vang conducted an unannounced inspection. The purpose of today's inspection was to deliver the finding regarding the above listed allegation. LPA met with Director Cristina Ortiz and explained the purpose of the visit. A tour of the facility was conducted, and a censue was taken.

During the investigation, LPA conducted two facility inspection and observed the classrooms. LPA interviewed staff members and daycare parents, reviewed facility records, and obtained copies of facility documentation. Evidence revealed that an incident occurred at the facility involving Staff #1 (S1) and Child #1 (C1). While S1 was in the process of changing C1’s diaper inside Classroom #4, C1 began running toward the door and S1 attempted to prevent C1 from leaving the classroom by grabbing C1’s shoulder, which resulted in scratching C1’s neck and leaving visible marks. This action was determined to be inappropriate for the handling of C1.

Based on the investigation, the preponderance of evidence demonstrates that a daycare staff member inappropriately handled a child in care; therefore, the above allegation is found to be SUBSTANTIATED.

Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, the following deficiency is cited during today’s inspection: (See next page, LIC9099-D).

Exit interview conducted and report was reviewed with Director Cristina Ortiz. Cristina was provided with appeal rights. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.

Director agrees to submit Plan of Correction (POC) to CCL-Fresno Office by 01/09/2026. The POC is to include proof of training materials pertaining to Title 22 Regulations-Children's Personal Rights and attendance (sign-in sheet) of staff participation.

During the complaint investigation, it was revealed that Staff #1 (S1) grabbed Child #1 (C1) by the shoulder while attempting to prevent C1 from leaving the classroom. This action resulted in S1 scratching C1’s neck and leaving visible marks. This posed a potential health, safety, and personal rights risk to children in care.

On 12/18/2025, Licensing Program Analyst, (LPA) Ka Vang conducted an unannounced inspection. The purpose of today’s inspection was to deliver the finding regarding the above listed allegation. LPA met with Director Cristina Ortiz and explained the purpose of the visit. A tour of the facility was conducted, and a census was taken.

During the investigation, LPA conducted two facility inspections and observed the classrooms. LPA interviewed staff members and daycare parents, reviewed facility records, and obtained copies of facility documentation. Evidence revealed that Child #1 (C1) had been scratched, hit, and C1’s hair was pulled by other daycare children while in the classroom or on the playground. However, the evidence did not establish that those injuries were the result of staff lack of supervision.
