25 | On 01/23/24, Licensing Program Analyst Denisia Jimenez met with Director, Beth Nelson for an unannounced case management inspection. LPA toured the classroom and took a census.
An Unusual Incident Report was submitted to the Fresno Community Licensing Office regarding an incident that occurred on 01/12/24, where a daycare child was left alone in the classroom for about 5 minutes. At the time of the incident there were 2 teachers and 20 day care children present in the classroom before walking out to the outside play area. Teacher #1 and teacher #2 had lined up the children inside the classroom by the front door before taking them outside and a head count was conducted. Teacher #1 was in the front of the line and teacher #2 was standing in the back on the side of the line and children and teachers walked out. Normally, once the children are outside of the classroom, the children are instructed to stand on the wall outside of the classroom and another head count is conducted. Unfortunately, this was not done and child #1 was left behind in the classroom. While teacher #1 was outside taking pictures of the children teacher #1 noticed child #1 missing and immediately informed teacher #2. The incident was observed by teacher #3 who had taken her classroom outside to the play area as well and happened to go back inside to get her clipboard. As teacher #3 was walking back outside to the play area she heard someone crying and walked into the classroom where the cries were coming from and saw child #1 standing alone in the classroom crying. Teacher #3 was walking outside with child #1 when teacher #2 was coming back to the classroom to look for child #1. Teacher #1 reported the incident to the Director who self reported the incident. LPA interviewed Director and staff present when the incident took place and determined that the child was left unsupervised for approximately 5 minutes. LPA spoke to the Director and reminded them to have visual supervision of children at all times whether they are outside or inside the classroom. The Director indicated that she immediately reached out to all staff and provided supervision training and reminded all staff of the importance of child supervision, keeping count of children at all times, and communicating with other teachers present when taking children outdoors. The Director also stated she will be conducting another training on 01/27/24. Although there was no immediate threat to child #1, it did pose a potential health, safety or personal rights risk to persons in care.
