25 | On 12/13/2024, Licensing Program Analyst Miguel Herrera conducted an unannounced Case Management Inspection. LPA met with Director, Beth Nelson. LPA toured the facility, and a census was taken. The purpose of today's inspection was regarding two different Unusual Incidents that were self reported to the Fresno Childcare Regional Office, the incidents occurred on 11/15/2024 and on 12/09/2024, both incidents pertained to lack of supervision.
During today’s visit LPA Herrera conducted facility observations, interviewed staff, and reviewed facility records pertinent to the two incidents. Regarding the incident that occurred on 11/15/2024, LPA Herrera determined that child #1 inappropriately touched child #2. Per staff interviews, child #1 was immediately redirected and then removed from the classroom, and the facility held a meeting with child #1’s parent. Teacher/child ratios are being maintained, and adequate supervision is being provided during this visit.
Regarding the incident that occurred on 12/09/2024, LPA Herrera determined that child #3 was accidentally left alone for approximately two minutes while the classroom was transitioning to the classroom across the hall. Although there was no immediate threat to child #3, it did pose a potential health, safety, or personal rights risk to persons in care. The facility was cited a Type B violation regarding this incident.
LPA spoke to Director Beth Nelson and reminded the facility to always have visual supervision of children whether they are outside or inside the classroom. Director Nelson stated that on 12/10/2024 the facility conducted a supervision training with all staff. The training covered topics such as the importance of child supervision, always keeping count of children, and communicating with other teachers present when taking children outdoors and indoors.
Exit interview conducted and report was reviewed with Director Beth Nelson. Appeal rights were provided. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, one deficiency was cited during today’s inspection (see LIC 809-D). This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
