On 12/14/23 at 11:25 am, an annual inspection was made to the facility by Licensing Program Analyst (LPA), Mendez. At 12:49 pm the home was toured inside and outside. The licensee and assistant were supervising 9 children, and operating within the licensed capacity and ratio requirements. The facility’s operating hours are 5am-5pm, Monday–Friday. The floor plan submitted by the licensee was reviewed and verified. The off-limits areas of the home are the upstairs, garage, basement door, and were made inaccessible by baby gate and lock. The children use the front yard as the outdoor play area and it is fully fenced. There were no pools or other bodies of water observed in the yard.

Eight children's records were reviewed at 11:34 am. Two staff records were reviewed at 12:11 pm. There are currently 2 adults living in the home.

The following deficiencies were cited assistant did not have current vaccines on file and current mandated reporter training certificate on file, licensee did not have proof of documented safe sleep logs for infants in care, licensee did not have safe sleep plan for infants in care.(see LIC 809D):
