25 | On 03/12/2024, at 9:30 am, Licensing Program Analyst (LPA) Michelle Hood conducted an unannounced case management inspection regarding a self - reported incident wherein a 4-year-old child was found unsupervised in the facility restroom, during the classroom transition to the playground. There were 16 children and two staff present on the day of the incident. LPA met with the Director. LPA discussed the purpose of the inspection and was led on a tour of the facility by the director.
During today’s inspection, LPA observed Classroom #1 with 14 children on the playground with 2 staff, Classroom #2 with 15 children on the playground with 2 staff, Classroom #3 with 11 children on the playground with 2 staff, and Classroom #4 with 14 children with 2 staff on the playground. In addition, the 2 year olds from Classroom #10, #11, and #23 were on there playground with 17 children with 6 staff. Classroom #5 there were 14 children with 2 staff, Classroom #6 there were 15 children with 2 staff and one staff shadowing a child, Classroom #7 there were 14 children with 2 staff, Classroom #8 there were 14 children with 2 staff, Classroom #12 there were 8 children with 2 staff, Classroom #19 there were 7 children with 2 staff, Classroom #21 there were 7 children with staff, and Classroom #22 there were 7 children with 2 staff.
On 03/12/2024, LPA interviewed the Director, 4 staff and 1 child. LPA reviewed video footage of the incident, and the staff & children's sign-in sheets. Based on staff interviews, and review of the video footage, one (1) Type A deficiency of California Code of Regulations, Title 22, Division 12, Chapter 1, is being cited on the attached LIC 809D.
LPA Hood informed Director Wendy Marquez that this report dated 03/12/2024 document one- Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk to the health, safety, or personal rights of children in care.
