25 | On 8/28/23 at 3:00pm, Licensing Program Analyst (LPA), Martha Malane arrived at the facility for the purpose of a follow-up pre-licensing inspection. Upon arrival, LPA met with Applicant, Leslie Valenzuela and was led on a tour the facility. Applicant submitted an updated LIC200A application to request a capacity of 19 infant children (age 12-24 months) and to withdraw the previously requested toddler option.
Applicant submitted an updated LIC999 facility sketch for the indoors and outdoors. The infant rooms measured a total of 684.27 square feet which is sufficient to accommodate 19 children. The nap room was not included in the square footage. There are two (2) toilets and two (2) sinks available for use. There is not a changing table within arm’s reach of a sink; applicant stated she will submit proof of a changing table within arm’s reach of a sink to the SDRO. The outdoor activity space measured a total of 1549.80 square feet which is sufficient to accommodate the requested capacity of 19 children.

· Current mandated reporter certification for applicant
· Updated approved fire clearance
· Changing table within arm’s reach of a sink
· Updated parent handbook
Exit interview conducted with Applicant, Leslie Valenzuela. A license for 19 infant children (age 12-24 months) may be granted after proof of the above requirements are submitted to the SDRO and a final file review. |
