25 | On 06/06/22, at 1:38 p.m., Licensing Program Analyst (LPA), Rajani Goudreau conducted an unannounced Annual Inspection and met with the Licensee, Graciela Chavez. LPA disclosed the purpose of the inspection and was granted entry into the facility by the Licensee. Nine children (three infants and 6 preschool age children) were in care with one staff member present. This facility is a one story, two- bedroom, three- bathroom house. The following areas are used for childcare: day care room (attached to house; located on the right side of house), restroom located in daycare room and backyard. The following off-limit areas are made securely inaccessible: two-bedrooms, bathroom located in master bedroom and bathroom located in the hallway of home. Operation hours are Monday through Friday from 6:00 a.m. to 3:00 p.m.
The fire extinguisher, smoke detector, and carbon monoxide detector met requirements, per LPA observation. LPA observed hazardous items inaccessible to children in care. LPA informed licensee poisons shall be placed in a storage area and locked. LPA observed toys, play equipment and materials available for the children’s use. Electrical fireplace located in the living room is securely screened. The home has a fenced backyard available for outdoor activities. LPA informed licensee children shall be supervised during outdoor activities. LPA did not observe any bodies of water on the premises during the inspection. In addition, licensee confirmed there are no bodies of water on the premises of the property. Licensee stated there are no weapons in the home. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Licensee’s First Aid and CPR certifications are valid and expire on July 2023. Licensee has required immunizations, per file review. Licensee has completed the Mandated Reporter Training. LPA informed the licensee the Mandated Reporter training shall be completed once every two years. The facility roster is maintained and was reviewed. See LIC809-C continuation page... |
