An Informal Office Meeting was conducted on May 30, 2025, at 1:00 PM at the Palmdale Regional Office. Present during the meeting were Licensing Program Managers (LPMs) Mariela Ramon, Claretta Yates, Lady King, Licensing Program Analysts (LPAs) Kuliema Calloway, Isabel Ortega, Mayra Rivera, and Licensee, Jaclyn Prado. The Informal Conference was conducted virtually via Microsoft Teams.

The goal of this meeting is to address recent concerns with the Operations of the Family Child Care Home and to assist the licensee to achieve and maintain facility compliance with Title 22 California Code of Regulations as it aligns with the mission of the Department.

During this Informal meeting, we addressed substantiated violation of Operations of a Family Child Care Home, Immunization's, Personal Rights, Reporting Requirements, and Staffing Ratio and Capacity with the licensee.

On January 8, 2025, based on investigative findings regarding the complaint allegation received December 23, 2024, the Department substantiated a violation of Immunization's- the Licensee accepted two children into the day care without having their required full immunization records this action did not meet the immunization regulations obligations. On February 12, 2025, based on the investigative finding regarding complaint allegations received January 7, 2025, the Department substantiated a violation of Staffing Ratio and Capacity- the Family Child Care is licensed for fourteen children. A review of the Community Connection for Childcare (CCCC) time sheets revealed from December 16, 2024, to December 20, 2024, there were twenty-three children in care and the care

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

dates were consistent weekly every day from September 2024 until December 2024 proving all twenty-three children were at the facility at the same time. The Licensee admitted they did have the twenty-three children in care and did not list the additional nine children on the facility roster as being in care because they were over capacity.

The Licensee received an additional substantiated violation on the same complaint for Personal Rights- the Licensee transported two-day care children in her vehicle without using the required booster seats and Licensee admitted they did not transport with the booster seats they were only using seat belts.

The following Regulations were reviewed and provided to the Licensee:

1. Staffing Ratio and Capacity


A large family childcare home may care for up to 14 children under specific conditions, including age and school enrollment requirements.
Licensee must notify parents when caring for more than 12 children.

2. Personal Rights
Children must receive safe, healthful, and comfortable care. Their personal rights cannot be waived.

3. Operation of a Family Child Care Home
A current roster must be maintained, including parent/guardian and physician contact details. This must be available to the Department upon request.

4. Reporting Requirements
The licensee shall report the following information to the Department by telephone or fax within the Department's next business day and during normal working hours (8am to 5pm).

5. Penalties
Repeat violations within a 12-month period may result in a $100 civil penalty per violation per day, up to 30 days.

In response to these discussions, the licensee has agreed to the following:
• Will not exceed licensed capacity.
• Will ensure proper use of booster seats when transporting children.
• Will provide a safe and healthful environment.
• Will maintain a current roster for three years and avoid creating new rosters with each new enrollment.
• Will always comply with all applicable Title 22 regulations.

Furthermore, the licensee agreed to complete and submit summaries for the following training videos by June 30, 2025, to LPA Kuliema Calloway at Kuliema.calloway@dss.ca.gov:
· Supervising Children in Family Childcare
· How many children can attend a Family Childcare Home
· Reporting Requirements
· What is a Civil Penalty?
· Children's Personal Rights in Childcare

· The licensee shall attend a Family Child Care Home Orientation and submit a copy of the Orientation certificate to LPA Calloway by 08/30/2025.
· The licensee shall attend a Family Child Care Home Orientation scheduled at the Palmdale Child Care Regional Office on August 26, 2025, at 9:00 a.m. and submit a copy of the Orientation certificate to LPA Calloway after completion.

A copy of this report must be given to the authorized representatives of all currently enrolled children, as well as to new enrollees for the next 12 months. It must be distributed no later than the next business day or the next day the child is in care.

The Acknowledgement of Receipt of Licensing Reports (LIC 9224) must be signed by the authorized representative and kept in each child's file. Failure to meet these requirements may result in a $100 civil penalty.

The licensee confirmed understanding of these requirements and had no further questions. An exit interview was conducted, and a copy of this report was read and emailed to the licensee.
