13 | On 08/18/2021 Licensing Program Analysts (LPAs) Araceli Gibson and Rene Mancinas, conducted an unannounced complaint inspection to provide findings regarding the above allegation. LPAs attempted to meet with Licensee, Candice Morris-Mossman. The purpose of the inspection was to discuss the above allegation and findings with licensee. LPAs were unsuccessful in reviewing and providing a copy of this report with Licensee.
During the course of the investigation, LPA Gibson inspected areas accessible to children in care, interviewed staff, children, and parents, and reviewed facility records. Investigation revealed the following:
During inspection of the backyard, which is an area accessible to children in care, LPA Gibson observed several items that could potentially pose a threat to health, safety, and personal rights of children in care. LPA Gibson observed large rocks, shovel, an uncovered propane tank, and potholes.
Based on the above observations the preponderance of evidence standard has been met, therefore the allegation that ‘Hazardous items accessible to day care children’ is SUBSTANTIATED. (Continued on 9099-C)
