On 02/10/2026, Licensing Program Analysts (LPAs), Nancy Her and Stephanie Vega-Gonzalez and Licensing Program Manager (LPM), Deborah Lowe conducted an announced pre-licensing inspection. Upon arrival, LPAs and LPM met with Applicant Bala Subbiah, current facility owner Gwen Haskins, and Director Traci Meyers. The purpose of this inspection is to follow-up on corrections from the 12/23/2025 pre-licensing inspection.

On 12/23/2025 the following corrections were needed prior to the issuance of the license:

· All pending completed documents that were requested via the Notice of Incomplete Application (NOIA) will be submitted.
· Playground Inspection for both the school-age and preschool playground climbers were needed and facility needed to obtain an age verifying sticker and submit proof of inspection reports to the Department.
· Facility would provide more sand cushioning for high-impact areas of all climbers for both the school age and preschool component.
· Both preschool and school age swings would need chain handle covers.
· Facility would repair all playground wrought iron fence doors to close to ensure the safety of children in care.
· Infant swings were in need of cleaning and infant safety straps needed to be replaced or repaired.
· Facility would repaint or repair all playground equipment that has peeling paint.
· Facility would add a separate sleeping area for the infant component

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

· Facility would provide the Department with a crib manufacturing manual to ensure they meet the current standards or provide new cribs for the infant component.
· Facility would obtain one potty chair to meet the needs of infants.
· Facility would work on dividing the Main Room and the Lunch/Snack Area to accommodate both the school age and preschool components as per discussion with Director.

During today's inspection, LPAs and LPM observed that these following items were corrected. The applicant provided the Department with a playground inspector report and that the two preschool climbers had age verification stickers from 2 years to 5 years of age. On today’s date LPAs and LPM observed that the swings were removed from the preschool playground and that there was appropriate sand cushioning on high-impact areas. LPA observed that the preschool equipment was painted. LPAs and LPM observed that the school-age outdoor climber was removed and longer present in the outdoor activity area. LPAs and LPM observed that the infant swings were removed from the infant outdoor activity area. LPAs and LPM observed that the infant room #5 had a separate sleeping and indoor activity space by a four-foot mesh partition wall. LPAs and LPM observed that the facility had obtained 12 new infants cribs that had appropriate mattress and fitted sheets. Facility obtained a potty chair to meet the needs of the infants.

LPAs and LPM remeasured the indoor space titled, Main Room and the Lunch/Snack Area, for both the preschool and school-age components, and the infant outdoor area. LPAs remeasured infant classroom number 5. The following are the current measurements for both indoors and outdoors for the infant, preschool, and school age components.



Infant Classrooms measurements are:
The Indoor total measurements contain a total of 1,286 square feet, which will accommodate Applicant’s request for 24 infants.
Preschool Classrooms measurements are:
The Indoor total measurements contain a total of 2,770 square feet, which will accommodate Applicant’s request for 75 preschool children.
School Age Classroom measurements are:
The Indoor total measurements contain a total of 862 square feet, which will accommodate Applicant’s request for 24 school age children.

Infant Outdoor Play measurements are:
Outdoor total measurements contain a total of 1,470 square feet, which will not accommodate Applicant’s request for 24 infants.
Preschool Outdoor Play measurements are:
Outdoor total measurements contain a total of 12,150 square feet, which will accommodate Applicant’s request for 75 preschool children.
School Age Outdoor Play measurements are:
Outdoor total measurements contain a total of 19,859 square feet, which will accommodate Applicant’s request for 24 school age children.

These are sufficient to accommodate an overall capacity of 24 infants; 75 preschoolers; and 24 school age. Applicant will submit an outdoor wavier for the infant component.

The following corrections are needed prior to the issuance of the license:


· Submit an updated LIC200A to match the updated capacity numbers.
· Submit an updated LIC 999 for the indoor and outdoor sketch for all components.
· Facility would place a four-foot division wall or partition to divide the Main Room and the Lunch/Snack Area to define the separate areas for the school age and preschool components.
· The applicant will submit an outdoor activity space waiver for the infant component. Currently the outdoor activity space can only accommodate 20 infants at a time. Applicants will submit a waiver request with an updated daily schedule.
· Applicant will place a gate to separate the preschool outdoor area from the school age outdoor area.

An exit interview was conducted with the above items discussed and a copy of appeal rights and this report was provided to the applicant. Applicant understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.

Final license determination will be made upon review by the Licensing Program Manager and correction of the above issues.
