Licensing Program Analysts (LPAs) J. Duarte and L. Xiong conducted the 10-day complaint investigation visit to the facility. LPAs met with Administrator Diana Diaz and informed her the purpose of the visit. During the course of this investigation it was discovered that the resident involved in the complaint does not reside at this facility. Therefore, the above allegations are UNFOUNDED.
