On 02/25/26, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct initial complaint investigation. LPA introduced self, stated the purpose of the visit, and met with Administrator Wendy Lee.

During the course of the investigation, the Department conducted interviews, records were reviewed and toured the facility. Facility has variety of activities provided for residents. Allegation alleging staff did not treat residents with respect and alleging staff are not ensuring activities are consistent, although the allegation may have happened or is valid. Based on interviews conducted, the preponderance of evidence standard has not been met, therefore, the above allegations are found to be UNSUBTANTIATED. Exit interview conducted. A copy of this report was provided to Administrator.
