25 | On December 22, 2021 at 10:50AM, Licensing Program Analyst (LPA) Brigitte Tsutaoka met with Applicant, Chunte Melvin, who guided LPA on a tour of the facility for a Pre-licensing inspection. This is a single story 2 bedroom, 1 bathroom home with kitchen/dining, and living room. The home used for day care is the Back House (BH) on the same property of the Applicant's home. There is no pool/spa or body of water on the premises. Family members residing in the home include 1 adult (applicant) and one child. Incidental Medical Services (IMS) policy was discussed.
Main care is provided in BH Bedroom #2 (bedroom directly at end of the hallway). Children use the bathroom in hallway (first room on the left). Off limits areas include the entire Front House (FH) (FH Bedrooms #1 through #3, FH Bathroom #1 and #2, FH Kitchen, FH Living Room, FH Dining Room, and FH Attached Garage), and BH kitchen. The home was inspected inside and out for safety, comfort, cleanliness, telephone service (land line), heating and ventilation, inaccessibility to poisons, detergents/cleaning compounds (locked under BH kitchen sink), medicines (FH Bathroom) and hazardous items (sharp knives locked under kitchen sink) that can pose a danger to children. Applicant reminded to conduct fire/earthquake drills every 6 months and document. A Roster was provided to Applicant to complete and maintain.
The backyard is completely fenced. There is an anchored outdoor play structure. LPA observed the tube slide have cracks and is in need of repair. Applicant agreed to remove slide and fence off opening to make safe for children in care before licensure. There is one dog that stays in FH with dog run made inaccessible by gate. |
