On January 13, 2026, at 11:30 a.m., Licensing Program Analyst (LPA) Isabel Ortega conducted an unannounced follow-up complaint investigation at Thomason Family Child Care. The LPA met with Licensee Anthony Thomason. The purpose of the inspection was to deliver the findings regarding the above complaint allegations.
On October 23, 2025, the Department received eight allegations regarding the Family Day Care Home. Each allegation was investigated. The investigation consisted of unannounced inspections conducted on 10/27/2025, 11/20/2025, 12/03/2025, and on today's day 1/13/2026. In addition, the investigation included confidential interviews with staff, children, and parents, as well as a review of facility records. The Licensee refused to allow the LPA to review home video recordings that included footage of the day care areas.

Based on the information gathered, the LPA received inconsistent statements regarding the eight allegations.
There was insufficient evidence to support that staff did not receive proper training. The Licensee denied that staff were ever left alone in the Family Day Care Home. The LPA observed valid, current EMSA approved Pediatric CPR certifications for each staff member present.
Allegation #2: There was insufficient evidence to support that Staff 6 (S6) yells at day-care personnel in the presence of children. S6 reported that personnel are corrected when observed doing something incorrectly, but not in a yelling manner.
Allegation #3: It was alleged that S7 is away from the home more than 20% of the time per day. S7 denied being away from the facility more than 20% of the time and stated that Staff 6 (S6) is always present at the day care when S7 leaves to run errands. Confidential interviews revealed that S6 is the main provider.
Allegations #4 and #5: It was alleged that day-care personnel engage in unsafe activities while children are present and engage in inappropriate conversations with children. S5 denied both allegations. There was insufficient evidence to support that staff were skateboarding instead of supervising children or engaging in inappropriate conversations. Confidential interviews disclosed that some conversations between staff and children involved video games and sports.
Allegation #6: It was alleged that the Licensee did not ensure day-care personnel obtained clearance prior to working in the facility. The LPA observed signed documents in staff files and reviewed the Personnel Clearance System, confirming clearance dates of association for staff.

Allegation #7: It was alleged that providers do not follow safe sleep regulations. LPA reviewed safe sleep logs and observed them to be complete.

Allegation #8: It was alleged that the provider did not ensure stairs were barricaded. During unannounced inspections conducted on 10/27/2025, 11/20/2025,12/03/2025, and today’s date 1/13/2026, the LPA observed a black iron child safety gate barricading the two steps.

Based on interviews, record reviews, and LPA observations, there was insufficient evidence to substantiate the eight allegations. Therefore, all allegations are deemed unsubstantiated at this time. Although the alleged incidents may have occurred or may be valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur.

An exit interview was conducted and a copy of this report, and notice of site visit was provided to Licensee.

License is was informed the notice of site visit shall be posted for 30 days visible and the report shall be made available to the public for three years.
