25 | On November 20, 2024, Licensing Program Analyst (LPA) Paul Garcia conducted an unannounced Case Management Incident inspection. LPA met with Director Vanessa Mendoza. LPA Garcia toured the facility indoors and outdoors and took a census. The purpose of today’s inspection was to discuss an unusual incident that was reported to Community Care Licensing (CCL) on October 7, 2024, where Child 1 (C1) sustained a head injury when child returned to his seat after dumping his tray during lunch time.
At approximately 1:30 PM on October 4, 2024, while C1 was returning to an assigned seat from discarding trach, C1 tripped over a nearby chair’s leg causing him/her to fall and hit his/her forehead on another adjacent wooden chair with enough force that caused an approximate one (1) centimeter laceration on his/her forehead above the right eye. S1 observed the incident and minimal bleeding, then immediately applied ice, gauze, and surgical tape to stop the bleeding.
The director notified child’s father of the incident and sent him photos of the injury however, child’s father chose not to pick him up until approximately 4:30 PM due to the lack of severity of the cut. C1 was taken to a medical provider later that evening around 8:30 PM and C1 received two stitches and was cleared to return back to care with no restrictions. Child returned to care on October 7, 2024. Stitches were removed approximately one week later.
LPA interviewed staff and analyzed the environment. LPA determined that this was an isolated incident that could not have reasonably been prevented however, moving forward, staff will have children push in their chairs at the table.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited. A notice of site visit was issued and must remain posted for 30 days. |
