25 | On 11/07/24, Licensing Program Analyst (LPA) Denisia Jimenez arrived at the facility to conduct an unannounced Case Management inspection. LPA met with Teacher/Director Claudia Barriga. LPA toured the facility inside and out and a census was taken.
The purpose of today's inspection was regarding an unusual incident that was reported to the Fresno Childcare Regional Office on 10/11/24. It was reported that on 10/09/24 the children were outside in the play area playing on the play structure. LPA observed the play structure has a slide attached to it and the children were running up the slide. LPA interviewed the Director. Child #2 waited to go down the slide and Child #1 came from behind fast and lost its balance. Child #1 fell from the top of the slide and stretched both arms out and its right arm bent backward. Director was standing across the slide and witnessed Child #1 falling. Director ran toward Child #1 and immediately attended to child and put an ice pack on its arm. Child’s guardian was contacted, and Child #1 received medical treatment. On 10/10/24 Director was notified that Child #1 has a cast on and will require surgery that is not scheduled as of today. Child #1 is back at the center with restrictions. Director is expressing the importance of using “walking feet” in the playground and will continue to remind the children to prevent further incidents.
Based on the observation of the play structure and interview with the Director, LPA did not see any concerns with supervision or ability to meet the needs or the health and safety or personal rights of children in care. This appears to be an isolated incident and staff took appropriate measures to address the staff, following appropriate policies, regulations, and reporting requirements.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.
Exit interview conducted and report was reviewed with Director, Claudia Barriga. Appeal rights were provided.
