On 4/9/2021, Licensing Program Analyst (LPA) Kathy Pacheco conducted an unannounced case management inspection at the facility and met with Director, Laurae Raths. The purpose of the inspection was to discuss Community Care Licensing (CCL) Regulations. During the inspection, LPA took a census, interviewed staff, and reviewed files.

Per California Code of Regulations Title 22, Division 12, Chapter 1, no deficiency was cited during today's inspection.

Exit interview was conducted with the Director. A copy of the Facility Evaluation Report (LIC 809) and the Notice of Site Visit (LIC 9213) were provided to the facility. LIC 809 is required to remain in the facility for public review and the LIC 9213 is required to be posted for 30 days.
