On 03/15/24, Licensing Program Analyst Denisia Jimenez conducted an unannounced case management inspection to discuss an incident report that was submitted to Community Care Licensing regarding an incident that occurred at the facility on 02/08/24. LPA met with Director Cheyenne Raths.

Cheyenne stated that on the date of the incident, Child #1 did not look good when they arrived at the facility. Interviews conducted indicated that during the day of the incident, preschool children had cereal for breakfast. Children were served Multi grain Cheerios. Child #1 is allergic to certain types of food and also has a skin condition. Staff #3 stated child did not eat their cereal or have anything else and just sat there and was in a different disposition. Staff #1 and Staff #2 assisted child #1 and took her to another classroom to get their temperature checked. Child #1 laid down on a mat while parent was called. Child #1 was picked up by parent. Child #1 was seen by their doctor and the child was administered and Epi-Pen and a steroid.

This appears to be an isolated incident and staff took appropriate measures to address the child’s incident following appropriate policies, regulations, and reporting requirements.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.

Exit interview conducted with Cheyenne Raths. This report is to be made available to the public upon request. LIC 9213 Notice of Site Visit to be posted for 30 days.
