25 | On 8/25/22, Licensing Program Analyst (LPA) Kathy Pacheco conducted an unannounced case management inspection. LPA met with Administrator Cheyenne Raths to discuss an incident that occurred on 7/14/22. LPA interviewed staff and observed area in which the incident occurred.
On 7/15/22, child (see Confidential Names Form (LIC 809) dated 8/25/22) was in the infant classroom. Staff (see LIC 809 dated 8/25/22) witnessed child crawling in a fast manner on the floor. Staff said child had a small soft toy in his hand and was sliding it on the ground as he crawled. Staff said the toy got stuck as he was crawling and caused child's arm to go underneath which made his head go forward and hit the edge of a small wooden cubby. Child had a bump near his eyebrow from the incident, but it has now gone away. Staff contacted child's parent immediately and referred parent to the ER. Child is okay and has returned to the facility.
Based on the information obtained, LPA determined Licensee/facility handled the incident correctly and reporting requirements were met. After interviewing staff, LPA determined Licensee/facility took appropriate measures to address child's injury. It was further determined Licensee/facility followed proper policies and procedures and no regulations were violated.
Per California Code of Regulations Title 22, Division 12, no deficiency cited during today's inspection.
A notice of site visit was given and must remain posted for 30 days.
Exit interview conducted and report was reviewed with Administrator Cheyenne Raths.
