On 06/07/2021, Licensing Program Analyst (LPA) Ruby Ocegueda, conducted an unannounced Annual Required Inspection for the preschool license. LPA met with Teacher Miguel Rodriguez who was substituting for Site Supervisor Angie Soto. Days and hours of operation are Monday through Friday 7:30 AM to 5:30 PM. This facility runs on a year-round schedule. LPA Ocegueda conducted a Covid-19 symptom screening today, provided identification, took a census and toured the facility.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. While inspecting the kitchen today, LPA observed about 3-4 knives in an accessible drawer. Also observed under the accessible sink cabinet were bleach, carpet spotter and two other unmarked liquids. There was a staff inside the kitchen, however the kitchen doors leading from room #1 and room #2 were not locked or had child safety mechanisms on the handles. LPA informed kitchen staff (staff #4) and Teacher Rodriguez of observations and both knives and disinfectants were observed to be made inaccessible. Teacher Rodriguez confirmed that the kitchen should not be accessible and would be discussing either keeping it locked or placing child safety cover on the door handles. No poisons were observed during the inspection.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Continue to 809-C

Drinking water is available both indoors and outdoors. While observing the playground area, LPA observed that there was a missing cup holder sleeve for the paper cups and some cups were touching the table. This observation was discussed with teacher Miguel Rodriguez. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. Also while observing the outside play yard, LPA observed a rod iron metal gate that was meant to keep the area behind a shed inaccessible. Today the rod iron fence was loose and made this area accessible to children in care. The area had leaves, a tricycle, and a wooden board. LPA discussed this observation with Teacher Miguel Rodriguez. The facility is free of flies, insects and rodents. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. The Designation of Facility Responsibility Licensing Form (LIC 308) was discussed today and teacher Miguel stated he would have one completed for himself as it was not observed during file review. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files. Staff #1 was missing LIC 503 (Health Screening) and current AB1207/Mandated Reporter Training. Staff #2 was missing LIC 503 (Health Screening) and required proof of Tdap and MMR immunization's. Staff #3 was missing required AB1207/Mandated Reporter Training. LPA advised teacher Miguel Rodriguez of file review observations and he stated that these documents might be in the Human Resources Office. LPA advised teacher to please ensure that all records pertinent to Licensing be at the facility ready for review. Menus are posted at least one week in advance where an authorized representative can view them. Continue to 809-C.

Incidental Medical Services (IMS) not currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. LPA and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

An exit interview with Teacher Miguel Rodriguez was conducted. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies were cited today.

This report shall be made available to the public upon request.
