25 | On 4/21/22, Licensing Program Analyst (LPA) Diana Martinez conducted an unannounced case management inspection to follow up on an unusual incident that occurred on the playground on the morning of 4/1/22. The incident was called in in on 4/1/22, and the Unusual Incident Report (LIC 624) was received by Fresno Community Care Licensing on 4/8/22. LPA met with Director Patty Gonzalez. The incident occurred when Child #1 (C1), who was running, lost his/her balance and fell. Upon falling, C1 hit his/her forehead on a lower bracket that was part of the play structure pole. Teacher-child ratio was in compliance when incident occurred. Staff #1 (S1) witnessed when child lost balance and fell as S1 was standing approximately 9-10 feet away. Child sustained a 1.50 inch long cut and first aid was administered. Parent and paramedics were contacted and child was taken to emergency room for treatment, which included stitches. On 4/13/22, the entire play structure was removed as it had been approved for removal by the Reef-Sunset Unified School District prior to this incident occurring. C1 returned to facility on 4/5/22, with the only restriction of staying out of the sun as per clearance from doctor, and is doing well. LPA observed the area where incident occurred and did not observe any tripping hazards as structure has been removed. Facility is in the process of installing a new play structure and area is fenced off making it inaccessible to children. This was an isolated incident and staff took appropriate measures to address the child’s injury following agency policies and procedures.
Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiency is being cited.
Exit interview conducted with Director Patty Gonzalez and a copy of this report was provided and discussed. A Notice of Site Visit Form (LIC 9213) was posted on parent's board and must remain posted for 30 days. |
