On 11/07/2025, Licensing Program Analyst (LPA) Octavia Nolan conducted an unannounced complaint inspection at the facility to deliver findings for the allegations listed above. LPA met with Licensee, Isabel Pardo and explained the purpose of the visit. LPA took a tour of the facility and a census.

During the course of the investigation, LPA conducted interviews with the Licensee and Staff #1 (S1), completed observations, reviewed facility records, and viewed videos posted to the Licensee’s daycare social media page.

Based on observation and interview, Licensee has separated children with a baby gate based on their developmental needs. Videos reflect the Licensee placed two children with developmental needs in an area that is fenced off from the other children. The video depicts the two children facing the other daycare children dancing. The two children were segregated from the group based on their needs.

Based on observation and interview, an infant in care has napped in an unsafe manner in feeding chair. The Licensee stated the child has a pattern of falling asleep in the feeding chair. LPA Nolan observed a child sleeping in a feeding chair placed on a couch, without any measures taken to prevent tipping in a video posted to the Licensee’s social media. Based on a review of records, the Licensee has not documented 15-minute sleep checks for infants in care. The Licensee stated she stopped completing the documentation after infants reached twelve months. The California Code of Regulations requires the Licensee to document the sleep checks until infants are 24 months. The documentation must also be available for Department review. LPA provided the Licensee with a copy of Infant Safe Sleep regulations.

Based on observations, documentation, and information gathered through interviews, the preponderance of evidence standard has been met, therefore the above allegations are SUBSTANTIATED.

Exit interview conducted and report was reviewed with Licensee, Isabel Pardo.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, the following deficiencies are being cited: (see next page LIC 9099-D).

Licensee was provided appeal rights.

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.

Licensee stated she will provide an updated written protocol on how she will ensure day care children's personal rights will be met and children in care are not segregated. This documentation will be sent to the Fresno South Child Care Regional office no later than 11/21/2025.
