A prelicensing inspection was conducted today at 12:30pm by Licensing Program Analysts (LPAs) Laura Chavez and Julie Gifford. LPAs met with facility representative Elizabeth Harkness. The applicant is requesting a license for preschool age children, with a capacity of 40. The facility will operate Monday-Friday, 7:30am to 5:30pm. The preschool is located in the Community Evangelical Free Church of Susanville. The facility has two rooms, Classroom B and C, three bathrooms for children and one staff bathroom.
The indoor and outdoor activity spaces were toured, and the facility sketch was verified. The following areas will be off limits to children: sanctuary, nursery, great room, utility closets, men's bathroom, and kitchen. These areas have been made inaccessible by means of baby gates placed in the door ways of Classroom B and Classroom C. There is a kitchen to store and prepare snacks. Children will nap on mats in each classrooms. The isolation area for sick children will be located in Classroom A. The changing table is located in the bathroom located to the right of Classroom B.

The outdoor play area is fully fenced. There is a climbing geometric dome, climbing slide structure and a modular climbing structure for children to play on and adequate cushioning underneath. The facility representative agrees that the facility will abide by age appropriateness of each climbing structure. There is no pool, spa, pond, fountain, or any other body of water on the premises. The facility representative agrees that children will be escorted to and from the outdoor fenced in play area.

The poisons are locked in the church janitorial utility closet. There is safe and age appropriate furniture, toys, and play equipment available for children.

A capacity worksheet was completed during the visit. There is enough indoor space for 44 children and outdoor space for 58 children. There are 6 toilets, 1 urinal and 5 sinks available for children. Based on the above the maximum capacity of the facility could be 44. Classroom A will be used as the isolation area for sick children.

The facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA reviewed with facility representative Elizabeth Harkness the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

The facility representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Exit interview conducted and report was reviewed with the facility representative Libby Markness.
