25 | On December 18, 2024, Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced case management inspection for the above facility. LPA was greeted by Director, Marjorie Golden, who guided LPA on a tour of the facility. The purpose of this inspection is to follow up on a self-reported incident that occurred at the facility on 12/09/2024. This is a preschool age program which operates Monday through Friday from 8:00 am to 4:00 pm. Facility consists of 2 buildings and each building consist of 3 classrooms. Per Director, there are 64 children enrolled. LPA observed 34 children in care.
Brief summary of incident: Child #1 (C1) was on standing on top of the steps of the playground structure which is on the far back of the playground. Child #3 (C3) and another child wanted C1 to play with them, so they pulled C1’s arms causing C1 to fall forward to the end of the steps. C1 landed and hurt her left arm. No medical attention was need.
During this inspection, LPA interviewed Staff #1 (S1), Staff #2 (S2), Child #1(C1) and Child #3 (C3). LPA attempted to interview Child 2(C2) who witnessed the fall but C2 refused to speak with LPA. LPA also observed the playground and play structure.
During the course of interviews, S1 and S2 did disclosed they were the only staff members on the playground during the time of the incident. Per S2, S1 and S2 were not fully on the playground yet when children were running up to them explaining what happened to C1. S1 explained that S1 was placed at the tables on the sand area, where S1 could not visually see children on the top of the steps of the play structure. S1, S2, C1 and C3 did confirm that a staff member was not near the play structure and not observed the C1 fall. LPA did observe children on the playground not being actively supervised. LPA observed staff predominately stay in one area and not providing 100% supervision for children in care on the playground. LPA observed multiple children on the play structure, which is on the far end of the playground, while one staff member was on the sand area closets to the patio near the door to the classroom and other teacher on the patio area. Leaving the children on the play structure unsupervised.
