25 | Licensing Program Analysts (LPAs) Nolan Tcheng and Suzana Safaryan conducted an unannounced Case Management inspection for the purpose for the following up on an unusual incident report. Upon arrival at 10am, LPAs met with Director Ellen Veselack, to whom the purpose of the inspection was explained. Tour of the facility was provided. There were children present during the inspection.
Census was taken. There were 102 children and 20 staff members.
On 09/28/2023, an unusual incident report was submitted to the Department regarding a child sustaining an injury on the facility grounds. Child was riding on the bike path and fell over, hurting their arm. Child sought medical evaluation and was found to have fractured right arm, LPAs conducted review of the play yard and obtained pictures of where the injury took place. Interviews were conducted with two staff members and one child. A staff member observed the incident occur and called for Staff #2 (Child's lead teacher) for assistance in attending to child. After review of information, there were no obstructions on the path or defects on the tricycles that could have caused the child to fall off the tricycle. Child was attended to immediately by staff, first aid was provided to the child, and an ouch report was created for the child on the day of the injury (obtained during today's inspection).
Based on the information obtained and observation of yard area, LPA determined that there was adequate supervision on the day of injury and it was not possible to prevent the injury from happening. Per California Code of Regulations Title 22, Division 12, no deficiency cited during today's visit.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
