25 | Licensing Program Analyst (LPA) Shushanik Safaryan conducted an unannounced Case Management Inspection for the purpose of following up on the incident reported to the Department . Upon arrival , LPA met with Facility Representative, Ellen Veselack, to whom the purpose of the visit was explained . Tour of the facility was provided . This is the preschool program and consists of 6 classrooms . There were 61 children and 15 staff members present during the inspection .
On 03/27/2024 , an unusual incident report was submitted to the Department regarding a child sustaining an injury on the facility ground . Child jumped on the teachers back when teacher was in process of standing up .Based on medical evaluation child was found to have dislocated elbow . LPA conducted interviews with Director , staff member and tried to talk to the child . Child was attended by staff, first aid was provided, and ouch report was created for the child on the day of the injury.
Based on the information obtained LPA determined that there was adequate supervision on the day of the incident , and it was not possible to prevent injury from happening .
Per California Code of Regulations Title 22, Division 12, no deficiency cited during today's visit.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
