25 | Licensing Program Analysts (LPAs) Nolan Tcheng and Mary Silva conducted an unannounced Case Management inspection to follow up on an incident report submitted to the department on 04/24/2023. Upon arrival at 11:40am, LPA was greeted by Director Christine Riddle, to whom the purpose of the inspection was explained. At 11:50am, LPA was provided a tour of the facility where there were children present during the time of inspection.
Census was taken. There were 26 children and 4 staff members.
On 04/19/2023, a police officer visited the facility for the purpose of investigating a cross report they received from Department of Child Family Services (DCFS). Facility reported the visit to the department as an incident report. The incident that the police visited for, that was reported as taking place on the facility grounds, is being addressed during this inspection. Child and parent made allegation against an adult, stating an incident took place at the facility.
During the inspection, LPA reviewed the location of classrooms, restrooms, and pick up/drop off locations. There are two classrooms that are adjacent to each other. There is a restroom that connects the two classrooms by a doorway on either side. Drop off and pick up typically take place by the gate on the playground. As well as the front of the school. LPAs obtained documentation in the form of a child care roster, facility sign in and sign out sheets, email threads, and contact information for additional persons (identified on attached LIC811). Interviews were conducted with three staff members. Pictures were taken.
A subsequent follow up visit will be conducted at a later date. No deficiencies are being cited during todays inspection.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
