32 | Per the licensee, there are no weapons, bodies of water on the premises. LPA did observe a small dog in home. Licensee obtains the current Pediatric First Aid and CPR certification which expires on 05/02/2026.
LPA conducted a children file review and deemed it not complete. 1 out of 5 children were missing a signed Parent’s rights (LIC 995), 1 out of 5 children were missing an Infant individual sleep plan (LIC 9227), 4 out of 5 children were missing immunization record for LPA to observe, 3 out of 5 children were missing 15 minute sleep logs. Per licensee, they were not aware they had to document infant safe sleep for infants in care. Licensee expressed that staff are always present and observing all children during nap time. LPA provided licensee a copy of all of the information that needs to be documents for infants safe sleep for children who are newborns to 24 months. LPA also advised licensee to read safe sleep regulations to ensure the facility is in compliance with title 22 regulations at all times
LPA conducted staff file review and deemed it not complete. 2 out of 4 staff could not provide proof that they have been immunized against influenza (or declination), pertussis, measles and TB, 4 out 4 staff could not provide proof of mandated reporter certificate.
All childcare employees must complete mandated reporter training within 30 days of hire and renew training every two years at www.mandatedreporterca.com
Licensee has the required documents posted in the FCCH; Facility License (LIC 203), Notification of Parents' Rights Poster (PUB 394) and Earthquake preparedness Checklist.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
