25 | Licensing Program Analysts (LPAs) Alicia Mooberry and Portia Bowden conducted an unannounced case management inspection due to an incident that occurred on 06/20/2024. LPAs met with Amy Nalasa, Director, who guided LPA on a tour of the facility.
There were 42 children supervised by 6 staff present upon arrival. LPAs and inspected the classroom toys, reviewed child #1's file and took photo of toy during this visit.
The purpose of the inspection was to follow-up on an incident that was reported to the department where Child #1 got their finger stuck in the hole a wooden track, the staff attempted to dislodge the finger and called 911 for assistance. No injury resulted from the incident.
The incident that occurred on 6/20/24, was reported to the Department on 06/20/24, via phone. The written LIC 624 was received by the department on 6/26/24. The facility is in compliance in Reporting requirements.
Based upon information received and observations recorded, the toys in the facility are age appropriate and in good repair. Per Director, Child #1 will be supervised while they play with the train tracks.
No deficiencies cited as a result of this inspection
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted with Emily "Amy" Nalasa, Director.
