A Plan of Correction (POC) visit was conducted on Friday, August 1, 2025, by Licensing Program Analyst Staicy Perry and Youle Qi to verify the correction of a Type A deficiency previously cited under Title 22 Regulation 101161(a) Limitations on Capacity. Upon arrival at the facility, Licensing Program Analyst (LPA) Perry and Youle were greeted by Director Pam. A tour of the facility was conducted. During the walk through, Director Pam did not allow the department access to the summer camp area. LPA was only permitted to tour the licensed preschool program. As a result, LPA was only able to obtain a census for the preschool program, which was 34 children in care at the time of the visit with 7 staff supervising the children.

However, LPA Perry and Youle observed that the summer camp program was still operating on the premises of the licensed preschool facility and Director Pam did not cease operation as indicated on previous report. LPAs did observe children to be in the summer camp and did observe parents dropping off children for summer camp. Director Pam stated that she is continuing to operate the summer camp program at the site and will not cease operation until August 8, 2025, which poses an immediate risk to the health and safety of the children in care. Per Director, she does not intend to cease operation of the summer camp until it officially ends on August 8, 2025, and acknowledged that she will accept any penalties the Department may impose due to the operation of summer camp program.

LPA conducted a consultation with the Director regarding the process of obtaining a school-age Child Care Center License. LPA Perry and Youle explained the required steps, including submitting an application, obtaining a fire clearance, and ensuring compliance with all applicable Title 22 regulations. Director stated she has considered applying for a school-age license, however, due to uncertainty surrounding the facility’s lease, she is unsure how much longer she will remain at the current location, estimating possibly one year. Director Pam indicated that once she receives confirmation regarding her ability to continue operating at the current site or if she will be operating and open for the following summer, she will collaborate with the Department to initiate the licensing process.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Director Pam needs to cease operation of the summer camp program immediately. Per Pam Director she will not cease operation until summer camp is over August 8, 2025 and accept whatever penalities will be accessed.

The licensee was found to be operating a summer camp program on the premises, serving 74 children between the ages of 6 and 12 years old, which is outside the licensed age range and capacity limitations of the existing preschool child care license.

The Director acknowledges that the LPA was not granted access to all areas of the facility during the inspection, specifically the summer camp area. The Director understands this is a violation of the Department’s authority as outlined in Title 22 regulations. Director will submit a declaration of her understanding of the departments inspction authority.

Additionally, LPA informed the Director that her website advertises the summer camp as if it is operating under the preschool license, which is misleading to the public. The Director acknowledged the concern and stated that it is an “easy fix” and she will make the necessary corrections. LPA discussed the applicable Title 22 regulations regarding advertisement and license limitations, and Director indicated understanding.


Based on observations, interviews, and a review of the facility’s conditions, it was determined that the facility continues to operate a childcare program beyond the limitations of its licensed capacity. This is a violation of Title 22 Regulation 101161(a) Limitations on Capacity. A repeat Type A deficiency is being cited today. Please refer to the attached LIC 809 and LIC 809D for the citation and Plan of Correction. A civil penalty is being assessed due to repeat violation. In addition facility is also receiving (one) Type A deficiency for 101200 Inspection Authority of the Department with civil penalties. This report documents (two) Type A deficiencies. A copy of this report must be posted in the facility for 30 consecutive days. A copy of this report shall be provided to all parents/guardians of currently enrolled children by the next business day or upon return if the child is absent. Any newly enrolled child for the next 12 months must receive a copy of this report upon enrollment. Each child’s file must contain a signed LIC 9224 Acknowledgment of Receipt.


An exit interview was conducted with Director Pam, and the following documents were provided to the director, Appeal Rights and Notice of Site Visit. Director Pam was informed that the Notice of Site Visit must remain posted for 30 days.
