NARRATIVE
1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	An unannounced in-person Case Management-Deficiencies inspection was conducted on this date by Licensing Program Analyst (LPA) Emiko Bell in order to cite for deficiencies observed during an inspection. Upon arrival, LPA was greeted and let into the Center by Assistant Director Brenda Solarzano, who got Director Liliana Vasquez, to whom the purpose of the inspection was announced. LPA and staff wore face coverings the duration of the inspection as a precautionary measure against COVID-19. The COVID screening questions were posed and all were responded to with "No." 10:35-Director Vasquez guided LPA to the Preschool classrooms; both classes were leaving to the playground. There was one staff and four preschool children in the Green Parrots Room and one staff and six preschool children in the Red Robins room. Staff-child ratio was met. It was upon noting the name of the staff in the Green Parrots Room and reviewing the "Facility Personnel Report Summary," that LPA noted that Monique Escobedo is not associated to the Preschool license; she is the opener for the Preschool and has been employed since 03/02/21. She was previously employed, cleared and associated to the Preschool license through 11/30/20 but was disassociated after leaving for other employment before returning. Upon further review of the LIS 531, LPA discovered that the Preschool closer, Gisselle Infante, who has been employed since 05/24/21, does not have Criminal Background Clearance through Community Care

Licensing at all. Director Liliana Vasquez logged onto Guardian and verified that neither have Criminal Background Clearance as well. A Type A citation and a civil penalty in the amount of $500 each (for a total of $1000) has been assessed,

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

LPA Emiko Bell informed Director Liliana Vasquez that this report dated 03/08/22 document(s) two Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Emiko Bell informed Director Liliana Vasquez to provide a copy of this licensing report dated 03/08/22 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview conducted and report was reviewed with Director Liliana Vasquez.
