25 | Licensing Program Analyst (LPA) Stephanie Li conducted a case management inspection for construction incident that was reported to licensing on 08/27/24 and construction began on 08/26/24. LPA met with Director Hilda O.-Koorn, who guided the analyst on a tour of the facility. LPA observed 11 preschool children and 3 teachers present during the time of inspection.
LPA conducted interview with Director, made observations of the roof and play ground. Per director, preschool roof was not scheduled to be repaired. LPA observed that the preschool side roof has stacks of tiles (10 in each stack) a foot apart all along the preschool ride side. Roof tile stacks are about 3 feet away from the ledge. The ledge slants down over the preschool play yard. Per interview with director, she was not aware that they were going to work on that side of the roof. LPA advised that the preschool children do not play in the yard until the roofing is completed. LPA advised for children to play in the covered breezeway outside of the children's classrooms. There are toys in the breezeway that were donated for children to use for the day.
Per director, construction began on Monday, 8/26/24 and will conclude by this weekend. LPA obtained an updated LIC624 to indicate modified outdoor play plan.
Director stated that the children will not be present tomorrow as the center is having a staff development day and the facility will be closed on Monday for Labor day. Per director, roofing will be concluded by Tuesday morning.
Based on information obtained during this investigation, no additional follow up is necessary regarding the incident reported. No deficiencies are being cited at this time.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Exit interview conducted and report was reviewed with Director Hilda O.-Koorn.
