Licensing Program Analyst (LPA) Roxana Lopez conducted a Case Management Deficiencies visit on this date to address deficiencies revealed during a POC inspection. Census was taken.

LPA arrived at 1:58 pm, walked in through the back of the facility and observed Director Elodia coming out of the infant facility. Director, took LPA to the office to set up- when entering the kitchen area in the preschool facility, LPA observed Staff # 1 clocking in.

LPA and Director walked into the 4-5 year old classroom, there was Staff # 2 out of ratio with 14 children, with children awake. LPA then proceeded to the 2's classroom, LPA observed Staff # 1 out of ratio with 13 children, with children awake. LPA did not observed any other teachers in the classroom and asked Staff # 1 who was watching the children before they clocked in from lunch. Staff # 1 disclosed that Staff # 2 was watching the 2's classroom as well. Director stated that they were in the 2s classroom but stepped out to the infants and asked Staff # 2 to watched both classrooms from the middle area in the hallway. This poses an immediate health and safety risk for children in care- civil penalties were assessed on this date. During the inspection Director stepped into the classrooms to be counted in ratio and combined some children for both classrooms to be in compliance.

Based on LPA observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety-

LPA Roxana Lopez informed Facility Representative Elodia Barnhart that this report dated 2/29/2024 document(s) 2 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Roxana Lopez informed the Licensee to provide a copy of this licensing report dated 2/29/2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

During the inspection Director stepped into the classroom for coverage and combined classrooms. Per Director- they are short staff- they called the sub list and were not able to get a sub. Director states that they are going to ask to hire a permanent sub- to have a back up. Written plan will be submitted
