An office meeting was held today with Elodia Perez Barnhart, Director, to review the facility's history, discuss substantial compliance with licensing laws and regulations, and provide the Directorilable resources.

The following deficiencies were reviewed: Responsibility for Providing Care and Supervision, Ratio, Criminal Record Clearance, and Poisons.

Responsibility for Providing Care and Supervision 101229(a)(1): (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.


- On February 22nd, 2024: LPA observed a child walking through an empty classroom with no supervision.
- On February 29th, 2024: LPA observed a teacher watching both the 2’s classroom with 13 children and the 4’s classroom with 14 children from the middle of the hallway.

Teacher-Child Ratio 101216.3(a): There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance.
- On February 22nd, 2024: LPA observed 1 teacher with 14 children in room 1(4’s) and 1 teacher with 13 children in room 2 (2’s).
- On February 29th, 2024: LPA observed 1 teacher with 14 children in room 1(4’s) and 1 teacher with 13 children in room 2 (2’s).

(1) Obtain a California clearance or a criminal record exemption as required by law or Department regulations:

On July 20th, 2023: One staff was present with a criminal record clearance.

Buildings and Grounds 101238 (g): Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. On August 5th, 2022: LPA observed poisons accessible to children in unlocked outdoor area storage.

To ensure compliance, the implementation of corrective and preventative measures was discussed. Technical Support Program was concluded on 7/26/2024. The Director states that they have completed the following training and/or corrections: They have hired additional staff to ensure ratio and supervision is being met. When a new staff is hired, Director makes sure that staff are not present in the facility until they have received clearance. Regarding poisons- Director is making sure daily that the storage are is closed and locked. Per Director they conducted a meeting with their staff discussing supervision, ratio and provided them with different resources.
Director will provide the following by 9/27/2024:
1. Provide a plan on how they will implement in- service training
2. Complete both Step 1 & 2 orientation and submit certificate
3. Submit meeting agenda and signatures
4. Updated LIC 500 Personnel Report

In-service training was discussed. CDSS videos for supervision and personal rights have been reviewed by the Director.

The Director was informed of the following:
1. Inspections of the facility will be increased to ensure compliance.
2. Responsibility to comply with Title 22 Regulations.
3. Advise to attend local licensing meetings and subscribe to provider updates.

Active Supervision At-A-Glance and a list of CDSS videos were provided and an exit interview was conducted with the licensee, who agreed with the topics discussed during today's meeting.
