An unannounced required inspection was conducted on this date by Licensing Program Analysts (LPA) Seung Lee. LPA met with Director Pam Deem who provided a tour the facility. Hours of operation are Monday through Friday, 6:30-6:00pm.

The infant component is located in Classroom B on the same site as both the preschool #191597334 and schoolage #198001785 components. The infant program has no shared areas with the other two components.

The infant room was inspected. AM snack, lunch and snack menu was observed posted near the classroom. There is a changing table within arm's reach of a sink. There are age appropriate toys, equipment, and furniture. LPA did not observe any hazards or violations in the classroom.

The outdoor playground was inspected. The infant playground is separately gated from the preschool playground. Outdoor equipment was inspected for safety, cushioning material, good repair and appropriateness. No hazards or accessibility to bodies of water was observed. Outdoor drinking water is made available to children via water bottles.

Staff and children’s records were reviewed for completeness. The Needs and Service plans were reviewed. Files were found to be complete. Sign in and out sheets were reviewed. Office is used as isolation area. Medication is stored in director's office.

LPA reviewed SIDs, Never Shake A Baby, and safe sleeping practices. Infants should sleep mouth up, on their backs, free of clutter surrounding their sleeping space. Copy of Safe Sleep Practices was provided on this date.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Currently there are no children in care with IMS needs.

No deficiencies were cited during today's inspection. Exit interview conducted with Director Pam Deem. A copy of this report and appeal rights were provided and explained.



The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
