25 | Licensing Program Analyst (LPA) Nolan Tcheng conducted a Case Management inspection for the purpose of following up on an incident reported to the Department on 01/14/2025. Upon arrival at 8:45am, LPA met with Site Supervisor Deanna Nancarrow, to whom the purpose of the inspection was explained. A tour of the facility was provided. Children were present during the time of inspection.
Census was taken. There were 37 children with 8 staff members.
On 01/13/2025, a child in care sustained an injury while on the play ground that would require medical evaluation. During today's inspection, interviews were conducted with two staff members and pictures were obtained of the area that the incident took place. During interview, Staff #1 stated that they observed the child fall as they were climbing down the play structure's spiral ladder. They stated the jacket the child was wearing was big and may have contributed to the child losing their balance. Immediate first aid was provided and family was contacted. Staff #2 confirmed that Staff #1 observed the child fall and was immediately their to respond to the incident. There has been no recent history of injury on this play area.
Staff interviews state they are more attentive in watching the children as they use the spiral ladder and position themselves below them now when they use it. Only one child at a time uses the ladder.
Upon review of the information above, Licensing staff determined that staff had provided adequate supervision and could not prevent injury from happening. Facility provided first aid, followed through with required steps, and reported the incident to the Department within the required 24 hours. No deficiencies are being cited in accordance with Title 22.
A notice of site visit was given and must remain posted for 30 days.
