An unannounced Required Annual site inspection was conducted by Licensing Program Analyst, (LPA) Bardo Baluyot who met with Site Director, Nancy Romero. Assistant Teacher, Raj Pierribia was also present during the inspection. LPA conducted a tour of all areas both indoors and outdoors and observed 5 children present with 2 staff. Required staff to children ratio was observed. Program currently operates morning/AM session from 6:30 AM to 9:30 AM and an afternoon/PM session from 1:00 AM to 6:00 PM. Staff and children's names were recorded. Care and supervision was evaluated to determine if the basic needs of children are met. LPA observed during this inspection that facility is in compliance of this.

Rooms identified on facility sketch were inspected. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Storage for children's belongings and an isolation area with a sink, toilet were inspected. Age appropriate sinks and toilets were inspected for availability and good repair. General sanitation was observed. Availability of indoor drinking water was observed. Emergency Drill Log was posted showing last earthquake drill was conducted on 3/17/22 and the last fire drill was conducted on 3/22/22. Fire Extinguisher label showed service date of 6/1/2021.

Outdoor area and equipment was inspected for safety, cushioning material, good repair and age appropriateness. Required shade, drinking water and fencing were inspected. Play area was inspected for hazards and inaccessibility to bodies of water.

Snack/lunch menus were reviewed. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness. A review of cleaning and food supply storage areas was made. Per Director, children have snacks from home or provided by the elementary school/district available to them.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. During visit no children or staff files were reviewed as they are kept in the main office: Options for Learning's main office, located at: 885 Village Oaks Dr. Covina CA 91724.

Site Director Romero was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A notice of site visit was given and must remain posted for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
