On 03/28/2024, at time 08:45 am ,Licensing Staff Shushanik Safaryan and Claudia Guangorena conducted an unannounced required 3-year inspection. A COVID-19 risk assessment was conducted prior to entering the facility. At 09:00 am Licensing Staff met with Facility Representative Diane Connell who guided Licensing Staff on a tour of the facility. This is a preschool age program with toddler option licensed for 44 preschoolers and 16 toddlers. There is an infant program (191598412) located on the premises that is physically separate from the preschool program both indoors and outdoors. This licensed facility is also located on a shared campus with Almansor Academy. Preschool program operates Monday through Friday from 07:00am to 05:30pm. Program consists of 2 preschool classrooms and 1 toddler classroom. Per Facility Representative there are 51 children enrolled.

Licensing staff observed all required forms/publications posted in each classroom. The following classrooms were inspected: Classroom 1(Toddler Classroom ): Licensing Staff observed 14 children with 4 staff members. Classroom 2( Ages 3-4 ): Licensing Staff observed 12 children with 3 staff members. Classroom 3:(Ages 4-5 ): Licensing Staff observed 15 children with 2 staff members. Teacher child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. Licensed facility is within the conditions, limitations, and capacity specified on the license.
During the tour Licensing Staff toured all the classrooms starting from the toddler class. All the classrooms furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All classrooms were inspected to ensure that the floors have a surface that is safe and clean. At 09:40 am Licensing Staff observed classroom #2 activity colorful carpet dirty with dark stains in multiple areas . Classroom #2 has a bathroom available for children use ,which is shared with the preschool classroom #3 . In the same bathroom there is a stall for the staff use. Licensing staff will review facility file to see is there any exemptions approved for the shared bathroom. All toilets and washing facilities are in safe and sanitary operating conditions.

In the preschool classrooms Licensing Staff observed emergency doors accessible to the children. One of the doors had a broken safety latch and the other one had a door lock which was not closed , which poses potential health and safety risk to the children in care. Licensing staff advised licensee to put a door alarm to prevent children exiting through that door unnoticed.

First Aid supplies are available and complete in each classroom.

Napping equipment and bedding was inspected for good condition, appropriate storage, and cleanliness. Storage for children's belongings was inspected. Parents take linens home on Friday and returned on Monday. During the tour LPA observed pile of blankets next to the cots . Per licensee blankets are not used for napping it is only used for activities and were put away to be washed .

During the tour Licensing staff observed children jackets piled up together and advised licensee each child’s item needs to be stored separately in their cubbies.


Licensing staff observed that there are functioning smoke and carbon detectors in the facility. Licensing staff observed a fully charged and serviced fire extinguisher, with service tag 06/2023. Emergency disaster drill was last conducted on 03/14/2023. Program Manager states there are no weapons, firearms, or bodies of water on the premises.

Snack menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity, and appropriateness to children in care. The facility provides breakfast and lunch. Per Facility Representative meals are provided by School district.

All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. Drinking water is readily available both indoors and outdoors, children bring their own water bottles too. The facility was observed to be free of flies, other insects, and rodents. The Directors office will be used for ill children and cot will be provided if necessary. Licensing Staff continued tour to the outdoor play area. Outdoor playground equipment is in safe condition, free of sharp, loose, or pointed parts. At 10:10 am Licensing staff observed cracks and holes on the surface of outdoor play area, which poses potential health and safety risk to the children's in care .

The surface of the outdoor activity space needs to be maintained in a safe condition and to be free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard.

At 10:51 pm Licensing staff observed 13 children with 4 staff members from Classroom #2 taking walk around the school. Per Licensee each child has parents’ permission slip signed for neighborhood walk . Licensing Staff reminded licensee during the walk staff need to make sure have the list with names of the children and the consent for medical treatment in case of an emergency .

Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Facility uses a program called Pro Care app child to sign children in and out. Facility has hard copies of sign in and sign out sheets as needed.

Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. Inspection of required forms was made and documented on the LIC 857. LPA reviewed staff records; staff files were complete. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. The review of Staff records was documented on the LIC 859.

Staff present had proof of the AB 1207 Mandated Reporter Training certificate on file. AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018, any person who works in a childcare facility shall complete the training and renew the training every 2 years. At 10:30 am Licensing staff asked staff member to open medication cabinet for review .Licensing staff observed that staff was unable to open medication box indicating other staff member change the code . Licensing staff had to wait for the other member to come back from the restroom break to open the medication cabinet , which poses immediate risk to the health and safety to children in care . During the review Licensing Staff observed child’s Epi pen without original box and without any labels which poses immediate health and safety risk to the children's in care.

Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Facility Representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.


For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance with the Written Directives outlined in PIN 21-21.1-CCP. Based on the report no lead exceedance was detected .

Based on the LPA’s observations and records reviews, the following deficiencies listed on the attached LIC 809D(Deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.

A notice of site visit was given and must remain posted for 30 days.
