25 | On 12/17/2024, at 1:05 pm Licensing Program Analysts (LPAs) Carolyn Tuba and Monica Ruiz conducted a case management inspection due to an incident that was self-reported and submitted by the facility. The incident occurred on 12/5/2024 at pick time approximately 2:00 pm. Covid risk assessment was conducted. LPAs met with Child Development Supervisor, Ryan Martinez who arrived shortly after, to facilitate the visit. LPAs observed and took a census of 13 children with 2 staff during naptime.
The incident was reported to the Department within the required 24 hours of occurrence.
During this investigation, LPAs conducted interviews with Staff #1 (S1), and Staff #2 (S2) and received via email an email from Child #1 (C1)’ parent. According to the Child Development Supervisor they conducted their own investigation and have since close the incident.
According to the Staff and Child Development Supervisor, C1 no longer attends this facility and has moved to another license facility within the Hacienda La Puente School District. C1 had stated that a S2 had hurt their arm. LPAs were not able to interview C1 due to not being available.
No citations have been issued at this time.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Child Development Supervisor, Ryan Martinez.
