25 | On 12/16/2021 at 08:10AM, Licensing Program Analyst (LPA) Denise Miranda conducted an unannounced follow-up case management inspection at Beverly Hills Presbyterian Church Nursery School located at 505 North Rodeo Drive, Beverly Hills, CA 90210 for the purpose of following up and deliver the final findings of the investigation of the unusual incident that was self-reported by the facility on 11/02/2021. The El Segundo Child Care Regional Office received the report on 11/06/2021. LPA met with Director Caridad Greco. There are 71 children present with 13 teachers and Director.
According to the incident report, on 11/01/2021, children were being dismissed for the day. Child#1 was walking up the stairs and slipped and hit his forehead on the step. There was a laceration approximately 1 inch in length. Staff cleaned the forehead of the child and applied ice. Parents were contacted to pick up the child and took to the Doctor. No stitches were required but the doctor placed a derma bond on child’s forehead. Per Director, child#1 returned to school next day on 11/02/2021.
During this inspection, LPA conducted interviews with staff and child involved. LPA toured the classroom area and stairs and observed it to be free of holes, broken glass and other debris that could pose a hazard. LPA observed the railing stair it was not loose or broken. During this inspection, LPA reviewed staff#1 and #2 record that were wittiness the incident. LPA also obtained a copy of the sign in and sign out sheet dated 11/01/2021 and children’s roster.
Based on the information obtained, it does not appear that this incident was the result of a Title 22 violation for lack of care and supervision. Although the child sustained an injury, staff #1 visually observed the incident and immediately applied ice on the child’s forehead. Parents were also notified immediately. An exit interview was conducted and a copy of this report along with the Notice of Site Visit were provided to Caridad Greco, Director. |
