Licensing Program Analyst (LPA) Doris Whitmore met with licensee, Carol Tatsumi, who guided analyst on a tour of the facility for a Random visit. Upon arrival LPA Whitmore observed 7 children in care and 2 staff supervising the children. This is a 1 story home in a residential community used as a commercial business. The facility operates Monday, Wednesday& Friday 9:00a.m. to 12:00p.m. & Tuesdays & Thursdays from 9:30a.m. to 12:00p.m. Children bring their own snack and water bottles. Water is also provided to the children.

The following areas are used for primary care. LPA observed the inside of the school. The school consist of 3 main care areas. LPA observed a music room, activity room and kitchen. The facility was inspected inside and out for safety, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents/cleaning compounds (in locked cabinet), and hazardous items (sharp knives top of locked cabinet) that can pose a danger to children. Facility roster complete and maintained current. Fire/earthquake drills current.

Children play in the backyard and front yard. There are large trees for shade and play structures for active play. LPA inspected equipment for safety. LPA observed age-appropriate activities and play equipment for children to play. The Outdoor has a combination of sand and straw/hay.

Per licensee, there are no weapons or firearms of any kind in the facility at this time. The LPA did not observe any weapons. The required fire extinguisher (2A10BC) and smoke detectors & Carbon Monoxide are in operable condition. LPA reviewed a total of 10 children files there were two files that were missing the Admissions Agreement. LPA observed California School Immunization Record on pink paper. LPA advised Licensee that she can use blue paper. LPA also gave Licensee information for L.A. County Dept. of Public Health Customer Support for ordering Blue Cards at (323) 869-8080. LPA observed 3 staff files One staff who was not present at the time of the inspection and was not on guardian.

LPA informed LPA that the forms have been completed there was no copy of the ID and the teacher would need to be associated to the facility before returning C PR/First Aid expire 6/27/23. Mandated reporter training certificates are all current 07/26/22.The First Aid kit was observed and is complete. Licensee has posted as required the License, Emergency Disaster plan and Parents Rights Poster.

The following was discussed with the licensee:


Mandatory Forms for the children’s files and provider’s files, Requirements for fire drills, earthquake drills and documentation for both. Role and responsibilities of being a mandated reporter were discussed. The licensee was advised how to access forms and Regulations for Family Child Care online at www.ccld.ca.gov . Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. The licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must have the facility’s phone number at all times; if the phone number is changed, licensing must be notified. Regulation prohibits the smoking of tobacco in a private residence licensed as a family child care home 7 days a week, 24 hours a day. State law prohibits baby walkers, bouncy seats, exer-saucers and any other items that fall into that category.

Requirements for fingerprint clearances and associations were discussed with the licensee. Licensee can be cited a civil penalty of $100 per day, up to $500.00 for the 1st offense and up to $3000.00 for the 2nd offense within a 12 month period. The licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC624B.

The licensee was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. If a serious violation is cited, a copy of the licensing report (LIC809 or LIC9099) must also be posted for 30 days. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed. Copies of the reports must also be provided to each parent when a serious deficiency, Type A, is cited (LIC9224).

Licensee was also encouraged to read the Child Care quarterly updates every season as the come out to stay informed of any changes or updates to the regulations. Licensing must have the facility’s phone number at all times; if the phone number is changed, licensing must be notified. Regulation prohibits the smoking of tobacco on the premises.
The licensee was informed of the responsibility to report suspected Child Abuse by calling the Child Abuse Hot line at 1-800-540-4000. Also call the CCL office within 24 hours of the Unusual Incident and follow up with a written Unusual Incident/Injury Report (LIC 624B) within 7 business days.

Mandated Reporter: Beginning on January 1, 2018, AB 1207, requires all licensed providers, applicants, directors and employees to complete General and Child Care Mandated Reporter training as specified on their mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training. The OCAP modules are free of cost and available at: http://www.mandatedreporterca.com/.

There were no deficiencies today 09/20/2022 An exit interview was conducted, and a copy of this report was given to Carol Tatsumil along with the Appeal Rights.
