13 | On 03/16/2023 at 8:40a.m. Licensing Program Analyst (LPA) Doris Whitmore conducted an unannounced visit for the purpose of delivering complaint investigation findings regarding the allegation above, LPA met with Kirstin MCDuffie and observed 25 children and 4 staff at the time of visit.On 01/09/2023 Licensing Program Analyst Doris Whitmore conducted the initial complaint inspection to the above facility
LPA met with the Director Kirstin Mc Duffie, who guided LPA on a tour of the facility. There were fifteen (15)children in care and four (4) staff upon arrival. During the investigation LPA interviewed, the Director, Staff, Childrenand obtained a copy of the Sign in Sheet for 01/09/23. Maintenance Request Form, Enviro- Master, PersonnelReport, Daily Classroom Checklist, Facility Roster, Orkin Bill, Academy for Early Learning Parent Handbook, and supporting documentation. In addition, LPA interviewed staff, children, and the Director. On LPA's 01/09/2023 visit , LPA observed several buckets in back classroom for the rain. Based upon observation and information obtained via interviews there are leaks in the front room, and infant room, of Licensee does not ensure the roof is maintained in good repair is substantiated. Meaning that the allegation is valid because the preponderance of the evidence standards has ben met. |
